    When faxing the paperwork, please be sure to include the following completed forms:

  • 1) Page 2 of the Walk-In Pay Services Agreement
  • 2) Page 7 of the Walk-In Pay Services Agreement
  • 3) Page 8 of the Walk-In Pay Services Agreement (fill in the lines under Agent)
  • 4) Authorization Agreement for Pre-Arranged Payments form (page 9)
  • 5) Copies of a voided check and a government issued picture identification
  • 6) Attachment A form with your store’s location information ( Include current email address ) (page 10)
  • 7) Sales Tax Certificate
  • 8) 30 days Current Bank Statement
  • 9) T-Mobile attachment if you are T-Mobile authorized
  • 10) Verizon attachment if you are Verizon authorized
  • * If you make a mistake, please cross out the error and initial next to it ( DO NOT USE WHITE OUT )
